Correcting a Birth Certificate

Correcting a birth certificate (most often a misspelling), which requires an affidavit, can be initiated at our office. Walk–in service is offered for these corrections. Once you have verified the information on the birth certificate and the information to be changed with our Registrar, the affidavit is then sent to the state so that the birth certificate can be updated.

Adding a Father to a Birth Certificate

Single, unmarried mothers wishing to add a father to a birth certificate can initiate the process at our office. Walk–in service is offered for this addition and requires that both parents be present. Our Registrar will complete an Acknowledgement of Paternity Affidavit, obtain both signatures, and then send the affidavit to the Central Paternity Registry and the State so that it may be updated.
