Obtaining a Certified Copy of a Death Certificate

Each certified copy is $25.00. Uncertified copies of death certificates are not issued.

Walk–In
Complete and print an application form and bring it to Galion City Health Department. Only cash, check, or money order is accepted for walk–in service. Office hours for this service are Monday–Friday 8am–4pm.

Mail Order
Complete and print an application form and mail it, along with a self addressed, stamped envelope and money order (only) for $25.00 to:

Mail orders are processed and sent out the same day they are received by the Health Department.
